"Empowering communities with relevant information, advice & suport to make confident and informed choices."
What We Offer
Everything we do is about making health and wellbeing easier to understand, easier to access, and easier to act on. We meet people where they are — in homes, churches, workplaces, and community spaces — and walk alongside them with care, clarity, and respect. Our work is grounded in two core pillars — Outreach and Public Health Promotion — that strengthen health literacy, improve access to services, and support the wellbeing of our communities.
OUTREACH SERVICES
We bring services into the community so people don’t have to face the health & social system alone.
Nursing Outreach Service
If you’re finding it hard to get in with your GP, our nurses are here for you. We offer health checks, early risk detection, and simple explanations so you know what’s going on and what to do next.
Open: Friday 10.00am - 2.00pm at 10 Scott Court, Stokes Valley
Plus pop‑up community clinics across the region — keep an eye out for us in your neighbourhood.
